ドイツの 32 の集中治療室における中央作成による教育的介入の中心ライン関連血流感染予防に対する効果を評価するための時系列分析

Time-series analysis to observe the impact of a centrally organized educational intervention on the prevention of central-line-associated bloodstream infections in 32 German intensive care units

背景
中心ライン関連血流感染(CLABSI)は予防策により減少するが、必ずしも実施されているわけではない。
目的
中央作成による教育プログラムがドイツの集中治療室(ICU)の CLABSI 発生率に及ぼす効果を調査すること。
方法
CLABSI 発生率が全国平均と同等またはそれ以上であるドイツの 32 の ICU を、277 の ICU および 67 の ICU を含む 2 つの対照群と比較した。予防策の実践状況および CLABSI 発生率の調査を 1 年間の介入プログラムの実施前、実施中、および実施後 2 年間に行った。各 ICU 内のクラスター効果を考慮した一般化線形モデルに基づく中断時系列分割回帰分析により、1 か月間の CLABSI 発生数と、時点、介入、およびその他の交絡因子との関連を推定した。
結果
合計で 266,471 中心ライン日に 508 件の CLABSI が認められた。ベースライン時のプールした平均 CLABSI 発生率は 1,000 中心ライン日あたり 2.29 であったが、追跡調査期間中は 1,000 中心ライン日あたり 1.64 まで有意に減少した。ベースライン時と比較した CLABSI 発生の相対リスクは介入期間中 0.88(95%信頼区間[CI]0.70 ~ 1.11)、追跡調査期間中 0.72(95%CI 0.58 ~ 0.88)であった。2 つの対照群ではいずれも変化はみられなかった。プログラムの実施が成功した後に、ICU における CLABSI 発生率の有意な低下が認められた。介入の実施に先行して CLABSI 発生率の低下がすでに認められていたが、研究への参加依頼と、包括的かつ多彩な研修資材の使用による CLABSI 予防に対する全般的な認識の向上が、実践に対する有益な効果を及ぼしたと考えられる。

監訳者コメント
CLABSI 予防マニュアルを中央で作成し、32 の ICU からそれぞれトレーナーを 1 日コースの教育プログラムで育成し、実際にこの予防マニュアルをそれぞれの ICU で実施したところ CLABSI の発生率が低下したことを示した研究である。1 つの病院でも実際には難しいこのような伝達型の教育が、より大規模なスケールでも有効であることを示した重要な研究である。本文中にはマニュアルの具体的な項目や、その遵守率の教育前後の変化なども紹介されており、参考になるだろう。
