Admission prevalence and acquisition of nasal carriage of meticillin-resistant Staphylococcus aureus (MRSA) in German rehabilitation centres

この多施設共同前向き研究では、ドイツの 11 のリハビリテーション施設の患者の入退院時のメチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）鼻腔内保菌について評価した。入院時に MRSA 保菌が認められた患者は 5,896 例中 71 例（1.2％）であった。MRSA 保菌の独立関連因子は MRSA 保菌歴（オッズ比［OR］4.3、95％信頼区間［CI］1.8 ～ 10.6）、過去 6 か月以内の入院（OR 2.5、95％CI 1.2 ～ 5.2）、ブタとの接触（OR 22.5、95％CI 11.1 ～ 45.7）、および慢性の創傷があること（OR 4.7、95％CI 1.9 ～ 12.0）であった。退院時に MRSA を新規に獲得していた患者は 0.3％であった。

監訳者コメント：
この論文によれば、ドイツのリハビリテーションセンター入院時の MRSA 保有率は 1.2％だった。周辺諸国の同様の施設での保有率（7.1％ ～ 14％）と比較するとかなり低い。さらに退院時に新たに MRSA 保有が判明したのが0.3％であり、高齢者リハビリテーション科の 26％よりも低かった。日本ではリハビリテーション施設入院時の MRSA 保有率はこれよりも高いと考えられ、入院中に曝露される機会も多くなる。さらに、多くの場合、急性期病院に比べて感染管理を担当する専従スタッフを含め体制が整いにくいこともあり、我々にとってもリハビリテーション施設での感染対策は大きな課題である。
