The sink as a correctable source of extended-spectrum β-lactamase contamination for patients in the intensive care unit

当院の集中治療室（ICU）の患者に 2010 年 12 月から 2012 年 4 月に、まれではあるが基質特異性拡張型 β-ラクタマーゼ（ESBL）産生菌の保菌が認められた。著者らはこれらの ESBL 産生菌が病室の流しに由来するという仮説のもとに、この問題を 20 週間にわたる毎週の患者および流しの培養により前向きに調査した。ESBL 産生菌は 13 台の流しから分離された。患者 4 例が、これまでに流しから分離された ESBL 産生菌と遺伝的に同一の ESBL 産生菌を保菌していた。このうち 1 例は ESBL 産生菌による肺炎で死亡した。自動消毒式サイフォンを ICU のすべての流しに設置することによって、流しから患者への伝播は終息した。

監訳者コメント：
流しから検出された ESBL 産生菌と患者から検出された ESBL 産生菌が遺伝子的に一致していた。すべての流しから菌は検出されたが、自動消毒装置をつけたら患者から検出されなくなった、という論文である。感染源は流しだとしても、感染経路を検討すべきでは、と疑問が残った。
