Surveillance and molecular characterization of non-tuberculous mycobacteria in a hospital water distribution system over a three-year period

アルバータ州南部の病院で、病院配水システム中の非結核性抗酸菌のサーベイランスを 3 年間にわたって実施した。研究期間中に、非結核性抗酸菌はいずれの取水サンプルにも認められなかったが、15 か所の給水サンプル 183 件中 106 件（58％）に認められた。2 種の非結核性抗酸菌、すなわちマイコバクテリウム・ゴルドナエ（Mycobacterium gordonae）（183 件中 88 件）とマイコバクテリウム・アビウム（Mycobacterium avium）（183 件中 34 件）が同定され、いずれも単一クローンであることが確認された。医療施設ではこれらの影響を受けやすことから、飲用水源由来の非結核性抗酸菌が患者の健康に及ぼすと考えられる影響を最小限にするように、注意を払う必要がある。

監訳者コメント：
病院配水システムの取水側と給水側（水道蛇口側）の非結核性抗酸菌の汚染状況を 3 年間にわたって調査した結果である。取水側から非結核性抗酸菌（NTM）が検出されておらず、2 種類ともに同一クローンであったことは、末端（蛇口）から汚染が逆行性に起こり、システム全体へ汚染が広がった可能性を示唆する。さらに、手動混合水栓が自動水洗によりも有意に NTM が検出されていることから、手動水栓での NTM 汚染の危険性を示唆している。しかしながら、一旦汚染されるとバイオフィルム形成により容易に除去することは困難である。NTM による院内感染の報告はまれではあるが、本研究の結果から、異なる病棟においても同一の配水システムで複数例が発生した場合には、集団発生（定着や感染症）の可能性も念頭におかなければならないことを示唆している。
