Point prevalence surveys of healthcare-associated infections and use of indwelling devices and antimicrobials over three years in a tertiary care hospital in India

インドでは、医療関連感染症、ならびにデバイス留置および抗菌薬の使用について定期的なサーベイランスを行っている病院はわずかである。本研究の目的は、インドの 1,800 床の大規模 3 次ケア病院における医療関連感染の有病率、ならびにデバイス留置および抗菌薬の使用率に関する 2 回にわたる 1 日時点調査を実施することであった。医療関連感染症の全有病率は 7％であり、手術部位感染症の割合が最も高かった（33％）。デバイス留置は 497 例（27％）で行われており、915 例（50％）が抗菌薬投与を受けていた。本研究は、研究対象病院における医療関連感染症の有病率、ならびにデバイス留置および抗菌薬の使用率に関する強固なベースラインデータの作成に有用であった。

監訳者コメント：
インドを含めた発展途上国では、医療関連感染に関するサーベイランスを行っている病院はまれである。本研究では 2008 年 9 月 5 日と 2011 年 9 月 23 日のそれぞれ 1 日ずつ、全病院での医療関連感染症やデバイス留置状況などを調査したもので、サーベイランスとしては簡易なものではあるが、それでもデータのほとんどない地域・国では価値のある情報である。また今後、何らかの感染対策を行っていくうえで、ベンチマークとして使用でき得る指標であろう。
それにしても、入院患者約 1,800 名のうち半分が抗菌薬を投与されているとは。入院患者のほとんどが感染症であるという発展途上国の特徴を表していると思われるが、抗菌薬の届出制などを行い始めたら担当者は大変であろう。
