Clinical and economic burden of surgical site infection (SSI) and predicted financial consequences of elimination of SSI from an English hospital

背景
手術部位感染症（SSI）は入院期間延長および費用の増加と関連することが知られているが、それらが外科手技の採算性に及ぼす影響については不明である。
目的
2 年間にわたる SSI による臨床的・経済的負荷を明らかにすること、および SSI 根絶の財政的影響を予測すること。
方法
2010 年 4 月から 2012 年 3 月に Plymouth Hospitals NHS Trust で大手術を受けた患者の SSI サーベイランスと Patient Level Information and Costing System（PLICS）のデータセットを統合した。主要評価項目は、SSI による術後入院期間、費用、および病院利益の相違（採算性）への影響とした。副次的評価項目は、すべての SSI を根絶することによる財政的影響の予測とした。
結果
SSI による入院期間延長の中央値は 10 日（95％信頼区間［CI］7 ～ 13日）であり、2 年間で合計 4,694 床日の逸失であった。SSI による費用増加の中央値は 5,239 ポンド（95％CI 4,622 ～ 6,719 ポンド）であり、研究期間中の超過費用の累計は 2,491,424 ポンドであった。全 SSI の根絶により発生する 2 年間の機会費用を算入した、全 SSI の根絶による財政的利益の推計値の合計は 694,007 ポンドのみであった。7 種類の手術カテゴリーでは、全 SSI の根絶に成功した場合は病院は財政的に悪化すると予測された。
結論
SSI は大きな臨床的・経済的負荷をもたらしている。しかし、現行の償還システムは SSI の減少に対する財政的な阻害要因となっていた。
