Outbreak of Stenotrophomonas maltophilia on an intensive care unit

背景
ステノトロホモナス・マルトフィリア（Stenotrophomonas maltophilia）は日和見感染を引き起こし、その多剤耐性のために依然として集中治療室（ICU）で問題となっている病原体である。
目的
ICU における S. maltophilia アウトブレイクについて報告し、汚染された飲料水給水器のリスクに焦点を当てること。
方法
疫学、環境サンプリング、および分子タイピングの併用により、アウトブレイクの分析を行った。
結果
2009 年から 2011 年に 23 例の患者から分離された S. maltophilia 分離株は 2 つの遺伝子型にのみ属していたのに対し、この期間の他の患者 52 例からの分離株は異なる菌株であることが明らかになった。すべての S. maltophilia 株の 1 か月間の発生率の範囲は 0％から 11％、2 つのアウトブレイク株は 0％から 9％であった。ICU 入室時および ICU での週 1 回の咽頭スクリーニングから、アウトブレイク株は ICU で獲得されたことが示された（ICU 入室から分離までの期間の範囲 3 ～ 90 日）。大半の分離株（74％）は気道由来であった。保菌が認められた挿管患者 12 例中 2 例（17％）のみが肺炎を発症した。環境サンプリングにより、ICU のキッチンの 2 つの流しと冷水器の飲料水に 2 つのアウトブレイク株が認められた。S. maltophilia は、カーボンフィルターから冷水器までの可撓管（flexible tube）、および冷水器からキッチンの流しの水栓までの可撓管の内腔にバイオフィルムを形成していた。この冷却水は、ICU 患者の飲料水および口腔ケア用水として使用されていた。アウトブレイク株は冷水器の撤去後に消失し、1 か月間の発生率は ICU 入室患者の 2％未満まで低下した。
結論
本アウトブレイク報告は、ICU 患者に飲料水を供給する機器内のバイオフィルムによるリスクに焦点を当てるものである。
