Catheter-related bloodstream infections caused by Gram-negative bacteria

2008 年から 2010 年に、グラム陰性菌によるカテーテル関連血流感染（CRBSI）が微生物学的に確認された患者を、それぞれランダムに選択した対照 2 例と比較した。症例 81 例（全 CRBSI 症例の 17％）と、その他の病原体に起因する CRBSI を有する対照 162 例を試験の対象とした。グラム陰性菌による CRBSI 発生率は、入院 1,000 件あたり 0.53 であった。症例は対照と比較して、神経系や消化管の基礎疾患、抗菌薬療法歴を有する割合が高く、血液培養が陽性となるまでの期間が短かった。現在の入院中の手術（オッズ比［OR］3.5）、緑膿菌（Pseudomonas aeruginosa）感染（OR 3.6）、菌血症の合併（OR 4.1）は、高い死亡率と関連していた。グラム陰性菌による CRBSI は全 CRBSI 症例の 17％に及ぶ、上述の特性を有する患者に対する経験的治療にあたっては、グラム陰性菌による CRBSI を念頭に置くべきである。
