背景
胸骨の手術部位感染症(SSI)は生命を脅かす恐れがあり、その発生率を低下させるためにあらゆる方策を講じるべきである。
目的
開心術後の胸骨 SSI 発生率について調査すること、および感染制御介入による発生率低下効果を明らかにすること。
方法
2005 年から 2012 年に胸骨切開術を受けた成人患者を対象として、胸骨 SSI の前向きサーベイランスを実施した。研究期間中の感染制御介入として、セファゾリンまたはバンコマイシン予防投与、胸骨 SSI のサーベイランスとフィードバック、手術前の黄色ブドウ球菌(Staphylococcus aureus)の鼻腔内スクリーニングとムピロシンによる除菌、メチシリン耐性黄色ブドウ球菌感染・保菌患者の隔離、手術前の血糖値の適切な管理、およびクロルヘキシジン/アルコールによる皮膚消毒などを、種々の時間間隔を置いて実施した。
結果
研究期間中の胸骨 SSI 発生率は 18,460 例中 479 件(2.59%)であった。胸骨 SSI の原因で最も多かったものは、コアグラーゼ陰性ブドウ球菌(CoNS)(36%)および黄色ブドウ球菌(31%)であった。感染制御介入により、胸骨 SSI 発生率は 2005 年の 3.63%から 2012 年には 1.65%に低下した(P < 0.0001)。
結論
本研究から、適切な感染制御介入によって胸骨 SSI 発生率の低下が可能であることが示された。しかし、実施した介入は黄色ブドウ球菌による胸骨 SSI 発生率の低下にのみ有効であった。黄色ブドウ球菌による SSI の制御に成功した病院では CoNS が大半の胸骨 SSI の病原体となっており、CoNS による胸骨 SSI を制御するための介入方法を早急に明らかにする必要がある。
