Successful control of an outbreak of colonization by Klebsiella pneumoniae carbapenemase-producing K. pneumoniae sequence type 258 in a neonatal intensive care unit, Italy

本稿では、イタリア・パレルモの新生児集中治療室（NICU）で発生した KPC 型カルバペネマーゼ産生肺炎桿菌（Klebsiella pneumoniae）（KPC-Kp）の配列型（ST）258 による保菌アウトブレイクについて報告する。KPC-Kp ST258 は積極的監視培養プログラムにより検出された。アウトブレイク期間中の 2012 年 9 月 18 日から 11 月 14 日に入院した新生児 54 例中 10 例から KPC-Kp が分離された。感染症例は認められなかった。男児であることは保菌と関連していたが、スルバクタム・アンピシリン＋ゲンタマイシン投与は保護的に作用した。新規入院に対するNICU 閉鎖の必要なしに、感染制御介入によって KPC-Kp の伝播は遮断された。

MLST 258 型のアウトブレイク事例である。日本国内での KPC 型カルバペネマーゼ産生肺炎桿菌の検出事例はこれまで少ないが、欧米諸国の広がり具合からすると注意が必要である。
