微生物付着およびバイオフィルム形成の予防のための医療器材表面の加工

背景
微生物の付着や生存を抑制する作用がある表面を有する器材の開発は、デバイス関連感染予防の有望な方法である。
目的
微生物の付着や増殖を抑制する作用がある表面を有する器材を設計するために用いられている戦略についてレビューを行うこと。
方法
公表文献の PubMed 検索。
結果
1 つの戦略として、殺生物剤で処理した医療器材を設計することが挙げられる。殺生物剤を素材に含有させる、コーティングする、または共有結合させることによって、殺微生物剤が放出されるか、あるいは放出されずに接触による致死がもたらされる。医療器材に殺生物剤を使用することには、細菌耐性のリスクや毒性の可能性といった問題のために異論がある。もう 1 つの戦略は、微生物の付着が防止されるように、素材表面の化学的または物理的性状を変えることである。微生物の付着は、微生物の生物学的構造や環境に直接的に依存する複合的な現象である。表面の化学的な抗付着加工は、多くの場合、素材の疎水性を標的としたものである。表明形状の加工で重要となる特性は粗度(roughness)とナノ構造であり、それらのサイズや空間的構造は制御可能である。今後は、細菌の付着を低減するうえで最も効果的な物理的パラメーターを特定する必要があり、これは細菌の形状やその他の特性に依存すると考えられる。
結論
殺生物剤の放出によらない、微生物の付着の低減に基づく予防戦略は有望である。表面加工をしたそれらの器材の臨床的有効性のエビデンスは不足している。どのような物理的性質が付着の低減に最も有効であるのかを明らかにするため、また抗菌薬以外の殺生物剤によるコーティングの有用性を評価するために、さらなる研究が必要である。
