重度のクロストリジウム・ディフィシル(Clostridium difficile)感染症に対するバンコマイシン単剤療法と併用療法の治療効果の比較

背景
重度のクロストリジウム・ディフィシル(Clostridium difficile)感染症(CDI)に対して推奨される治療法は経口バンコマイシン単剤である。メトロニダゾールとの併用療法はショック、イレウス、または中毒性巨大結腸症が合併した場合にのみ推奨される。しかし併用療法を支持するデータが不十分であるにもかかわらず、重度の感染症患者が併用療法による治療を受けることが多い。
目的
重度の CDI 患者に対する経口バンコマイシン単剤療法と併用療法の転帰の相違を評価すること。
方法
経口バンコマイシン単剤療法または併用療法を 72 時間以上受けた重度の CDI 患者 78 例のカルテの後向きレビューを実施した。主要評価項目は CDI の臨床的治癒までの期間(定義は、合併症が発生せずに、下痢の消失が 48 時間以上持続した最初の日までの期間)とした。その他の評価項目は治癒率、合併症発生率、および再発率とした。
結果
臨床的治癒率は単剤療法と併用療法との間に相違はみられなかった(57.1%対 65.1%、P = 0.49)。臨床的治癒までの期間中央値は単剤療法群 7.0 日、併用療法群 8.0 日であった(P = 0.19)。想定される交絡因子で補正後の、臨床的治癒までの期間の併用療法の単剤療法に対するハザード比は 0.58 であった(P = 0.10)。再発率および個々の合併症の発生率は両群間に相違はなかったが、全合併症発生率は併用療法群で有意に高かった。
結論
これらのデータから、重度の CDI に対する単剤療法と併用療法の転帰には相違がないことが示唆された。

監訳者コメント:
メトロニダゾールは炎症病巣への移行はよいが、改善すると悪くなることが知られている。したがって、重症例では経口でのバンコマイシン投与が第一選択薬となる。
