Obesity and nosocomial infections

背景
医療関連感染の予防は、現代の医療の主要な目標である。内的な患者側の因子は、医療関連感染リスクに寄与していると考えられる。
目的
肥満と、医療関連感染のリスクおよび転帰との関連について調査すること。
方法
肥満と院内感染、および肥満と抗菌薬投与に関連する研究の PubMed 検索。検索語は、「肥満（obesity）」、「感染（infection）」、「院内感染（nosocomial infection）」、「手術部位感染（surgical site infection）」、「救命救急部門（critical care unit）」、「菌血症（bacteremia）」、「尿路感染（urinary tract infection）」、「医療関連感染（health care associated infection）」とした。
結果
複数の研究により、肥満は医療関連感染リスクの増加と関連することが示されていた。この関連は、特に心臓、血管、整形、および消化器の手術で明確に認められた。体格指数（BMI）のデータは、外科的・侵襲的手技を受ける患者では記録頻度が高かった。BMI データの記録方法は文献ごとの一貫性がなく、多くの研究では対照群または基準群（正常体重）の BMI 中央値でさえも過体重や肥満を示していた。したがって、感染リスクの増加がみられる BMI の明確なカットオフ値を明らかにすることはできなかった。肥満では多くの場合、医療関連感染の予防と治療のいずれにおいても抗菌薬が過少投与されていた。肥満は抗菌薬の薬物動態に影響を及ぼすことを示唆する研究がある。しかし、肥満での抗菌薬投与に関する推奨方法はない。
結論
肥満は院内感染リスクを上昇させ、多くの場合、医療関連感染の予防と治療のいずれにおいても抗菌薬の過少投与と関連していた。肥満の増加へのわれわれの対処能力が、今後の病院衛生における予防的活動の主要な課題である。

この研究には様々な交絡因子が絡んでいる可能性がある。肥満と糖尿病などの患者背景は易感染性患者や創感染との関係が気になるところである。
