背景
クロストリジウム・ディフィシル(Clostridium difficile)感染症(CDI)患者に対しては、個室が使用できない場合はコホーティングが推奨されている。患者は下痢の症状が消失した後も感染性が持続している場合があり、コホーティングを継続することによって患者の再感染リスクが上昇する可能性がある。
目的
CDI 再発のリスク因子を特定すること、および患者のコホーティングが再発リスクの上昇と関連するかどうかを明らかにすること。
方法
2008 年 10 月から 2011 年 6 月に当院で CDI と診断された患者 248 例を対象として、患者の人口統計学的特性、併存疾患、CDI の重症度、および治療内容が記録されたデータを収集した。主要評価項目は診断後 30 日以内の症状再発とした。
結果
CDI 患者 138 例(55.6%)がコホート隔離病棟に入院した。これらの患者は、非コホーティング患者と比較して重度の CDI の割合が高く(オッズ比[OR]1.95、95%信頼区間[CI]1.10 ~ 3.46、P = 0.022)、バンコマイシン投与を受ける割合が高かった(OR 1.59、95%CI 0.94 ~ 2.68、P = 0.083)。再発は 26 例(10.5%)にみられた(コホーティング患者 21 例、非コホーティング患者 5 例)。入院時の尿路感染症(OR 5.16、95%CI 2.10 ~ 12.64、P < 0.001)、コホーティング(OR 3.77、95%CI 1.37 ~ 10.35、P = 0.01)、抗菌薬併用投与(OR 2.07、95%CI 0.91 ~ 4.72、P = 0.083)が再発リスクの上昇と関連していた。多変量解析では、コホーティング(OR 3.94、95%CI 1.23 ~ 12.65、P = 0.021)および尿路感染症(OR 4.27、95%CI 1.62 ~ 11.24、P = 0.003)が有意な再発予測因子であった。
結論
C. difficile のコホート隔離病棟への入院患者は再感染リスクが高いため、再発リスクが上昇していると考えられる。C. difficile の制御ためにコホート隔離病棟を使用する病院は、このリスクを最小化するために、コホートにおける患者のフローの管理を行うべきである。

監訳者コメント:
なんとも驚きの結末である。こうなると CDI 患者は個室隔離をして、かつ退出後には次の患者に二次伝播しないようにするために徹底した病室清掃消毒が必要ということになる。
