Assessing data sources for sustainable and continuous surveillance: surgical site infections following coronary artery bypass grafts in England

背景
医療システムが欧州各地で収集している国や地域レベルのデータソースの中には、冠動脈バイパス術（CABG）に関する情報も含まれている。英国では現在、この患者集団の手術部位感染（SSI）のサーベイランスは義務化されていない。
目的
英国で現在利用可能な、CABG 施行患者の SSI サーベイランスのためのデータソースについて調査・比較すること。
方法
Cardiothoracic Surgery in Great Britain and Ireland（SCTC）Adult Cardiac Surgery Registry、英国健康保護局の SSI サーベイランス、および単一の大規模な英国国民保健サービス（NHS）トラストの Patient Administration System からデータを抽出した。患者識別コードを用いてこれらのデータの確定的な関連づけを行い、匿名化したうえで記述的分析を実施した。
結果
2011 年 1 月 1 日から 2011 年 6 月 30 日に、患者 306 例が CABG を受けたとして 1 つ以上のデータセットに記録されていた。これらの患者のうち 76％は調査対象とした 3 つのデータソースすべてに記録されていた。それ以外の記録状況が一貫していない患者では、5％がSCTCレジストリと Patient Administration System に、18％が英国健康保護局のサーベイランスと Patient Administration System に記録されていた。合計 28 件の SSI が記録されており、このうち 3 つのデータベースすべてに記録されていたのは 21％であった。
結論
現在、CABG 施行患者に関するデータを収集・蓄積しているデータベースには、重複や不一致が認められる。効率的で持続可能な電子サーベイランスシステムを構築するためには、不一致が認められる複数のシステムを統合することが推奨される。

この論文では、3 つのデータベースで収集されている CABG 施行患者のデータを、すべての患者が網羅されているか、データに抜けがないか、データは正しく収集されているかについて検討し、いくつかの問題点を明確にした。
日本でも、施設単位、学会、地域、国レベルで様々なサーベイランスが行われている。結果から対策や施策を導き出すためには信頼できるデータが必要であり、サーベイランス自体の評価は極めて重要である。
ちなみに、サーベイランスの評価の際、求められる因子には以下のようなものが挙げられる。簡便性、柔軟性、受容性、感度、陽性的中率、代表性、迅速性。
