Reduction in postoperative endophthalmitis with intracameral cefuroxime

術後眼内炎は白内障手術の合併症として、発生頻度は低いが深刻な転帰をもたらす。本報告では、イングランドの地域総合病院での 8 年間にわたる眼内炎発生率の動向と、この動向を改善するための試みについて述べる。2007 年に眼内炎アウトブレイクが発生したため詳細な調査を実施し、それに続いて治療方針の変更を行った。これまでのセフロキシム結膜下投与に代わって、セフロキシム前房内投与を導入した。「ペニシリンアレルギー」を有する患者へのセフロキシム前房内投与に関する調査を行い、安全であることが確認されたため、治療指針を変更した。その結果、眼内炎の発生率は 4 分の 1 に低下した。
