Otitis externa following aural irrigation linked to instruments contaminated with Pseudomonas aeruginosa

背景
外耳道の感染症である急性外耳道炎の発生率は、オランダの一般開業医（GP）の診療所では 1 年間で患者 1,000 例あたり約 14 件である。2010 年の初めに著者の 1 人が、自分の診療所の外耳道炎患者の一部が数週間前に耳洗浄による耳垢除去を受けていたことに注目した。使用器具から採取したサンプルの細菌培養により、耳洗器の緑膿菌（Pseudomonas aeruginosa）汚染が認められた。それ以降、患者の耳からの緑膿菌分離株は検査室に保管していた。
目的
同一の診療所内で、汚染耳洗器の使用によって患者間の緑膿菌交差汚染が発生し得るかどうかを評価すること。
方法
診療所、病院耳鼻科外来、および Out-of-Hours GP Service の合計 17 施設で、外耳の検査および洗浄に使用した器具のスワブ採取を行った。器具からの緑膿菌培養株、および同一施設の患者からの分離菌株の遺伝子型判定を実施した。
結果
器具の汚染が認められた 4 診療所で、患者と耳洗器から検出された緑膿菌株との間、または同一の診療所の他の患者からの株との間に、遺伝子型の相同性が確認された。
結論
耳洗器から患者へ緑膿菌が伝播し、その結果外耳道炎が発生することが明らかとなった。著者らは当院の感染制御部門との共同により、診療所で実施すべき再利用可能な耳洗器に関する適切な洗浄・消毒・保管の推奨事項を作成した。
