背景
これまでの感染制御プログラムのコスト算出の試みでは、経済上のコスト（economic cost）ではなく会計上のコスト（accounting cost）に注目する傾向があった。会計上のコストを用いる研究は算出値が極めて粗雑であり、真のコストを過小評価している可能性がある。いかなる介入においても最大のコストの 1 つは職員の労働時間であるが、このコストは定量化が困難であり、これまでの研究ではほとんど無視されてきた。
目的
病院を単位とした感染制御介入またはプログラムのコストを検討・評価すること。また本稿では、介入コスト算出の際に考慮すべきいくつかの問題点についても論じるとともに、これらの問題を克服するための戦略を提案する。
方法
医療経済と心理学の両分野に関する既報文献や技術についてレビューを実施し、結果をまとめた。
結果
本稿では、コスト算出に関する包括的かつ利用が容易なガイドラインを提示した。また、研究対象の明確化や高額なコスト項目の重視などの主要な事項、およびピットフォール（過信や不確実性など）について論じた。
結論
今回の新規ガイドラインを使用することによって、病院職員やその他の研究者が感染制御プログラムまたは介入のコスト算出をより正確に行うことが可能となる。

監訳者コメント：
概念的な論文で少しつかみ所がないように感じた。結果的に、感染サーベイランスはコスト効果の高い感染対策の手段であると結論づけている。
