Quality control of the surveillance programme of ICU-acquired infection (ENVIN-HELICS registry) in Spain

背景
院内感染サーベイランスレジストリの正確性を確保するためには、データの検証は不可欠な要素である。
目的
スペインにおける集中治療室（ICU）感染に関する全国サーベイランスプログラム（ENVIN－HELICSレジストリ）を対象とした、初の質コントロールプログラムの結果について報告すること。
方法
2008 年にデータベースに登録された 13,824 件のカルテのうち、20 の ICU の 1,500 件（10.8％）の登録データを調査した。これらの ICU はランダムに選択したものであり、レジストリの登録症例数によって層別化した。感染患者の割合は 9.6％（95％信頼区間［CI］ 8.09 ～ 11.16）であり、調査対象の症例の選択期間中は一定であった。本研究のために 2 名の医師が研修を受け、調査を行った。
結果
ICU 在室中に何らかのデバイス関連感染が認められた患者の特定に関する ENVIN－HELICS レジストリの全体的な感度、特異度、陽性的中率、および陰性的中率は、それぞれ 86.0％（95％CI 80.0 ～ 92.0）、98.7％（95％CI 82.19 ～ 93.6）、87.9％（95％CI 82.19 ～ 93.6）、および 98.5％（95％CI 97.8 ～ 99.2）であり、κ 指数は 0.85（95％CI 0.79 ～ 0.92）であった。二次血流感染では最も感度が低く（59.3％）、挿管関連肺炎では最も感度が高かった（86.3％）。
結論
レジストリの登録担当者が報告したデータと監査者が検証したデータとの相関は良好であり、ENVIN－HELICS レジストリの信頼性が確認された。

過去の 1,500 件の ICU 患者のデータを用いて、サーベイランスの質を評価した論文である。その結果、感染症の定義の解釈のミスやプロトコール逸脱が見つかった。評価を行うことで、データ収集のエラーや脆弱性を見つけ出すことが可能となる。
