Prevention of surgical site infections in orthopaedic surgery and bone trauma: state-of-the-art update

整形外科および骨外傷での手術部位感染の予防には、その他の外科領域とは異なるいくつかの特徴がある。すなわち、少量の病原体によるインプラント関連感染、コアグラーゼ陰性ブドウ球菌やその他の皮膚共生菌の病原性、血行由来感染の可能性、長期にわたる退院後サーベイランスの必要性などである。整形外科での多くの手術部位感染予防策のうち、強固なエビデンスに基づくものは一部に過ぎず、どの要素が他の要素よりも優れているかを示すエビデンスも十分ではない。これらのことは、退院後の積極的サーベイランスや治療プロセスの各段階での予防策などを含む、集学的アプローチの必要性を示している。このようなアプローチは、個々の患者レベルでの術前管理から術中・術後管理に及ぶものであり、すべての医療関連感染を対象とした介入や抗菌薬管理の改善など、診療部門全体にわたる介入が含まれる。待機的整形外科手術での手術部位感染は、理論的にはゼロまで減少させることが可能であるが、現実的な減少が最大でどの程度かについては不明である。
