Outbreak of Ochrobactrum anthropi endophthalmitis following cataract surgery

術後眼内炎は重大な視力障害に進展することがある。本稿では、白内障手術後のオクロバクトラム・アンスロピ（Ochrobactrum anthropi）眼内炎のアウトブレイクについて述べるとともに、今後の症例発生リスクを最小限にするための新規滅菌プロトコールを提案する。O. anthropi 眼内炎患者、またはアウトブレイク期間中に本疾患を示唆する臨床所見を認めた患者のカルテレビューを行った。2010 年 7 月 24 日から 11 月 10 日に、O. anthropi による白内障手術後眼内炎患者 7 症例が確認された。O. anthropi 眼内炎アウトブレイクの原因として最も可能性が高いものは、水晶体乳化吸引装置のチューブの汚染であった。新規滅菌プロトコールの導入後に 1,000 回を超える白内障手術を実施したが、さらなる症例は発生しなかった。

眼科で使用する器具・器材の多くは粘膜に直接触れるものであるが、構造や素材による理由で高水準消毒や滅菌が困難なものも少なくない。加えて精密な器具が多いことからシングルユースが進まず、現場では対応に苦渋している。さらに、点眼薬が複数の患者に使用されるシーンもある。たとえ感染源が明確に特定されなくとも、アウトブレイク事例の報告を積み上げることがリスクの啓発になると考える。
