Outbreak of multidrug-resistant Pseudomonas aeruginosa infection following urodynamic studies traced to contaminated transducer

コリスチンのみに感性を示す極度薬剤耐性緑膿菌（Pseudomonas aeruginosa）による尿路感染症の院内アウトブレイクについて報告する。尿流動態検査後の患者 3 例に感染が認められた。3 例中 2 例は小児であり、このうち 1 例は尿路性敗血症も発症した。調査により圧トランスデューサの汚染が発見された。遺伝子型判定により、患者と圧トランスデューサからの分離菌株は同一であることが確認された。これらの圧トランスデューサは、汚染された尿が逆流して器材内部の液体と混ざる可能性があったにもかかわらず、「使用は 1 回限り」の表示がなかった。再利用可能な尿流動態検査器材と使い捨ての器材のいずれを用いるべきかという問題についての考察も行う。

監訳者コメント：
本論文のディスカッションによると、圧トランスデューサ本体には「再滅菌不可」と表示されており、「シングルユース」とは記載されていなかったが、発売元のホームページには同一の製品に対して「シングルユースに限る」と示されていた。不明瞭な表示も問題だが、汚染の可能性が想定されれば、再利用や消毒・滅菌についての対策を講じることはできたのではないだろうか、という疑問が残った。
