Gastroscopy-associated transmission of extended-spectrum beta-lactamase-producing Pseudomonas aeruginosa

2011 年 5 月から 8 月に、フランスの教育病院に入院中の患者 4 例から、まれな多剤耐性緑膿菌（Pseudomonas aeruginosa）が検出された。4 例全員が、5 か月の期間中に同一の胃内視鏡を用いた食道胃十二指腸内視鏡検査を受けていた。この内視鏡の培養の結果は、多剤耐性緑膿菌陽性であった。内視鏡洗浄の観察により、合意された手順からの逸脱として以下が確認された。不十分な初期洗浄、浸漬時間およびブラッシング時間の短縮、チャンネルの不十分なフラッシング、および保管前の不十分な乾燥。この内視鏡の使用を中止し、合意された手順の厳守を実施した後は、多剤耐性緑膿菌が分離される患者はなかった。

監訳者コメント：
緑膿菌による内視鏡の汚染事例、アウトブレイク事例はこれまでにも報告されてきた。器具を介した感染伝播のほとんどは、使用規定やマニュアルを遵守しなかった結果として発生している。再発を防止するための対応策、つまり、洗浄手順の遵守やリークテストなどのメンテナンスを確実に行うことはすでにわかっているのに、実践することは難しいことにジレンマを覚える。軟性内視鏡や超音波心エコー装置の場合、細かな亀裂にも問題となる菌の温床となる場合があり注意が必要である。
