Predictors of hand hygiene compliance in the era of alcohol-based hand rinse

背景
手指衛生遵守の予測因子の評価は、アルコール製手指消毒薬の時代である今日にはまだ行われていない。
目的
手指衛生遵守の予測因子を、アルコール製手指消毒薬の時代にあらためて評価すること。
方法
カナダの教育病院で、手指衛生遵守を直接観察法により 2 年間にわたってモニターした。遵守の標準化した定義を用いて、遵守の予測因子であると考えられるデータを記録した。クラスターを補正した一般化線形混合モデルを作成し、手指衛生遵守の予測因子の影響を評価した。
結果
3,487 名の医療従事者の 7,364 回の手指衛生機会を観察した。手指衛生遵守率は 45％であり、経時的な変動はみられなかった。多変量解析で示された遵守改善の予測因子は手指衛生を実施すべき機会などであり、体液への曝露後（オッズ比［OR］ 4.7、95％信頼区間［CI］ 3.7 ～ 6.1）および患者との接触後（OR 3.9、95％CI 3.5 ～ 4.4）は、患者との接触前と比較して遵守率が高かった。手袋の着用は、高い遵守率との関連がみられた（OR 1.3、95％CI 1.1 ～ 1.4）。職種が非看護師・非医師であることは、低い遵守率と関連していた（OR 0.72、95％CI 0.61 ～ 0.86）。1 時間あたりの手指衛生機会数は、低い遵守率との関連がなかった。石けんと水の使用頻度に対してアルコール製手指消毒薬の使用頻度が高い病棟では、遵守率が高かった（P ＝ 0.035）。
結論
アルコール製手指消毒薬の時代においては、手指衛生機会の頻度が高いことは、もはや至適な手指衛生遵守の達成のための主要な障壁ではない。しかし、アルコール製手指消毒薬の使用頻度が病棟により異なっていることは、依然として改善の対象であると考えられる。
