本研究では、クロストリジウム・ディフィシル（Clostridium difficile）検査のプロトコールにおける判定結果がグルタミン酸脱水素酵素陽性・酵素免疫測定法（EIA）による毒素陰性であった 102 件のエピソードについて調査した。これらの 102 の便サンプルのうち 46％の培養結果が毒素産生株陽性であり、その後 2 日から 32 日以内に 9 サンプルが EIA による毒素陽性となった。このようなデータは、こうした患者を症状が消失するまで別室で隔離し、他に説明のできない持続性の下痢を認める患者の治療を奨励するという当院の方針に一致するものである。第 3 の検査法（毒素産生性検査培養または PCR 法）を追加することが望ましいと考えられるが、その感度は各種の毒素産生性検査培養法や PCR 法によって大きく異なるようである。

監訳者コメント：
2012 年に英国の公衆衛生部門が発行したクロストリジウム・ディフィシルの診断と報告のためのガイドラインでは、2 つの方法を組み合わせた検査方法が必要とされている。また、近年米国では PCR 法による診断が主流となりつつある。日本では毒素の EIA 法が主流である。感染管理においては、検査診断が必ずしも優先されるものではなく、臨床症状による迅速な対応が感染拡大を防止するうえで重要であろう。
