Community-associated meticillin-resistant Staphylococcus aureus carriage in hospitalized patients in tropical northern Australia

背景
地方のオーストラリア先住民居住地域から、市中獲得型メチシリン耐性黄色ブドウ球菌（community-associated meticillin-resistant Staphylococcus aureus；CA-MRSA）が初めて報告された。これはオーストラリア北部で優勢な臨床病原体であり、病院環境内で伝播している可能性がある。
目的
Royal Darwin Hospital 内の黄色ブドウ球菌保菌の疫学的特性を明らかにすること。
方法
入院後 48 時間以内に登録した「入院直後の患者群」と入院後 5 日以降に登録した「入院患者群」の 2 群の患者を対象として、スクリーニングを実施した。黄色ブドウ球菌分離株の特性を抗菌薬感受性試験により評価し、複数部位塩基配列タイピング（MLST）に基づく高解像度融解曲線解析法により遺伝子型判定を行った。
結果
黄色ブドウ球菌保菌率は入院直後の患者群 225 例では 30.7％、入院患者群 201 例では 34.8％、MRSA 保菌率はそれぞれ 2.2％、18.9％であった。CA-MRSA は入院直後の患者群の 0.9％、入院患者群の 10.4％から分離され、医療関連（HCA）-MRSA はそれぞれ 1.3％、9.0％から分離された。入院患者群で最も多くみられた MRSA 株は、病院感染型 ST239 であった。入院直後の患者群で多くみられた CA-MRSA の clonal complex（CC）は 1 種類（CC5）、入院患者群では 7 種類（CC1、93、5、6、30、75、88）であった。
結論
入院患者に複数の CA-MRSA 株の保菌が認められたことから、病院内では典型的な HCA-MRSA 株だけでなく、多様な CA-MRSA 株についても選択圧が働いており、また伝播が生じていることが示唆される。

MLST は国や地域の MRSA 流行株の特徴をとらえるのによく使用されている。最近では施設内感染に関与するタイプの型や clonal complex も明らかになりつつある。
