Incidence and risk factors for hospital-acquired Clostridium difficile infection among inpatients in an orthopaedic tertiary care hospital

この後向き研究の目的は、整形外科患者における病院獲得型クロストリジウム・ディフィシル（Clostridium difficile）感染（HA-CDI）のリスク因子を特定することである。32 例の各 HA-CDI 症例に、対照例を 2 例ずつマッチさせた。発生率は 1,000 患者日あたり 0.33 であった。単変量解析により、入院後 24 時間以降の手術、抗菌薬治療、およびプロトンポンプ阻害薬が HA-CDI と関連することが示された。多変量解析では、入院後 24 時間以降の手術が HA-CDI と関連していた。手術に先立って入院した患者では HA-CDI のリスクが高かったことから、外来において術前の患者状態の最適化を適時に行うことによって、C. difficile への環境曝露の減少が可能であることが示唆される。
