Outbreak of skin and soft tissue infections in a hospital newborn nursery in Italy due to community-acquired meticillin-resistant Staphylococcus aureus USA300 clone

背景
市中獲得型メチシリン耐性黄色ブドウ球菌（community-acquired meticillin-resistant Staphylococcus aureus；CA-MRSA）は、世界各地で健常者の重度の市中獲得型感染症の原因となっている。
目的
イタリア北部の病院の新生児室で 2010 年 9 月から 10 月に発生した CA-MRSA アウトブレイクと、その調査および制御対策について報告すること。
方法
アウトブレイクの疫学について記述した。調査として新生児、両親、および病院職員の MRSA 保菌のスクリーニングなどを実施した。また、MRSA 分離株の分子タイピングを行った。
結果
新生児 9 例でアウトブレイクが発生し、このうち 3 例は重度の感染症であった。さらに、母親 4 例が産褥乳腺炎、母親 3 例および父親 1 例が皮膚感染症に罹患した。アウトブレイク株は CA-MRSA USA300 クローンであった。新生児、両親、および病院職員にアウトブレイク株の無症候性保菌が認められた。病院の適切な感染制御対策によりアウトブレイクは終結した。
結論
著者らの知る限り、本稿は CA-MRSA USA300 クローンによる病院アウトブレイクに関する欧州で初めての報告である。感染制御対策の強化、特に新生児などの高リスク群を対象とした感染制御対策の強化が、欧州の病院における USA300 株の流行を予防するために重要である。
