Current limitations about the cleaning of luminal endoscopes

背景
蛋白質などの患者由来成分が内視鏡内腔に存在・蓄積することによって、毒性作用、器材の損傷、不適切な消毒・滅菌、バイオフィルム形成リスクの上昇、病原体の伝播などの重大な影響がもたらされる可能性がある。
目的
単回の汚染･洗浄サイクル中の軟性管腔内視鏡チャンネルへの蛋白質の沈着と除去を評価すること。
方法
生検チャンネル内腔に接触する使い捨て内視鏡鉗子の汚染レベルを評価した。内視鏡検査室での観察後、管腔内視鏡チャンネル内部への蛋白質吸着に影響する因子および現行の初期洗浄法の操作について、蛋白質性の試験用汚染物質および高感度エピ蛍光顕微鏡法を用いて評価した。
結果
内視鏡の使い捨て部品は内腔汚染に関与していると考えられ、管腔内視鏡チャンネルを通過した蛋白質性汚染物質量の有用な指標であった。製造者の推奨に従った酵素洗浄およびチャンネルのブラッシングは、単回汚染後の新品の内視鏡チャンネルから蛋白質性残留物をすべて除去できるほど有効ではなかった。汚染直後のすすぎのみが、汚染除去の結果をわずかに改善した。
結論
現行の汚染除去法の効果は限定的であること、およびチャンネルの清浄度を患者間で評価するための質管理法がないことは、内視鏡検査中の有害な微生物や分子の交差感染リスク上昇の一因となっている。
