Patient narratives of surgical site infection: implications for practice

背景
患者の経験に関する調査は、診療を改善するために医療の中で広く行われている。しかしこれまでは、手術部位感染症（SSI）のサーベイランスや予防介入などにおいては、このような患者からの情報提供はわずかであった。
目的
診療の改善を図るため、患者の SSI の経験から情報を得ること。
方法
イングランドの 3 病院の SSI 患者 17 例（深部 SSI 4 例、臓器・体腔 SSI 12 例、および表在性 SSI 1 例）を対象として口述面接を実施し、次いで主題内容分析を実施した。
結果
患者は SSI の全体的な認識、関心、および理解が欠けていた。7 例の患者は自身が SSI であることを知らず、患者の説明から判断すると病院職員が患者に SSI について伝えていないこと、または SSI の存在を軽視したことが認識の欠如に関与しているようであった。プライマリ・ケアに多くの資源が投入されており、また 6 例の患者は 2 ～ 4 か月間休職していた。
結論
患者は SSI に対する関心が低いが、これが向上すれば予防介入の遵守が改善すると考えられる。本研究から、SSI の症状を把握するために患者の自己評価式の退院後サーベイランスの質問票を使用することの妥当性が確認された。またコストに関して、プライマリ・ケアのみならず、休職などの患者側にかかわるコストや、地域のコストなどを総合的に特定する必要性が示された。
