Control of multi-drug-resistant Acinetobacter baumannii outbreaks in an intensive care unit: feasibility and economic impact of rapid unit closure

アシネトバクター・バウマニー多剤耐性株（multi-drug-resistant Acinetobacter baumannii；MDRAB）による院内アウトブレイクが、2006 年 1 月から 5 月に総合的外科集中治療室（SICU）で発生した。このアウトブレイク中に、20 例の患者に MDRAB の同一株の保菌が認められた。制御策の導入にもかかわらず、アウトブレイクは SICU の完全閉鎖によってようやく終結した。2009 年 1 月に同じ SICU で 2回目のアウトブレイクが確認された際には、可及的速やかに SICU の閉鎖を実施した。この方策によりアウトブレイクの迅速な制御が可能となり、感染患者は 7 例にとどまり、25 日間で終結した。このアウトブレイクによる経済的影響もかなり小さく、推定費用は 2006 年の 539,325 ユーロに対して 2009 年は 202,214 ユーロであった。本研究から、SICU の迅速な閉鎖と患者の他所へのコホーティングは、MDRAB アウトブレイク制御のための費用対効果の高い方法であることが明らかとなった。

監訳者コメント：
アシネトバクター・バウマニー多剤耐性株によるアウトブレイクを探知したら、すぐに ICU を閉鎖することがアウトブレイクの終息に効果的であった、という論文である。2006 年と 2009 年のアウトブレイクの菌株は異なるものであったというが、なぜ、感染が拡大したのかという疑問が残る。貴重な機会ととらえ、しっかり疫学調査を行ってほしい。
