Significant reduction in vancomycin-resistant enterococcus colonization and bacteraemia after introduction of a bleach-based cleaning–disinfection programme

背景
当院では標準的なバンコマイシン耐性腸球菌（VRE）制御ガイドラインを遵守しているにもかかわらず、VRE 保菌・感染が増加している。
目的
漂白剤による洗浄・消毒プログラム「Bleach-Clean」を含む集学的な改善プログラムを病院全体で実施した。VRE の保菌、感染、および環境汚染を実施の前後で比較した。
方法
プログラムの内容は、新規製剤の導入（次亜塩素酸ナトリウム 1,000 ppm ＋ 洗浄剤）、標準化された洗浄・消毒法の実践、洗浄管理者の雇用、およびアルコール製剤による手指消毒と袖なしエプロン（長袖ガウンと手袋に替えて）を用いる修正プロトコールなどであった。VRE の分離には発色酵素基質寒天培地または標準的な検査法を用いた。評価項目として高リスク病棟（集中治療室、肝移植科、腎臓病科、血液内科）の VRE 保菌率（スクリーニング患者 100 例あたり）、環境汚染率、および病院全体の VRE 菌血症エピソードなどをプログラム実施の 6 か月前および 12 か月後に評価した。
結果
新規 VRE 保菌率（スクリーニング患者 1,948 例中 208 例から 4,035 例中 324 例へ 24.8％減少、P ＝ 0.001）および環境汚染率（66.4％減少、P ＝ 0.012）の有意な減少が認められたが、入院時の保菌患者の割合には変化がなかった。高リスク病棟の入院患者の VRE 保菌率も減少した（週あたりの入院患者の保菌率中央値 19.4％から 17.3％へ、P ＝ 0.016）。病院全体の VRE 菌血症発生率はスクリーニング患者 2,935 例中 14 例から 6,194 例中 5 例へ減少した（83.1％減少、P ＜ 0.001）が、バンコマイシン感性腸球菌菌血症には変化がなかった（P ＝ 0.54）。
結論
Bleach-Clean プログラムは、病院全体の高リスク患者の新規 VRE 保菌、および VRE 菌血症の大幅な減少と関連していた。これらの知見は VRE が流行している医療環境での VRE 制御に重要な意義を有すると考えられる。
