Feasibility and effectiveness of an electronic hand hygiene feedback device targeted to improve rates of hand hygiene

手指衛生遵守のモニタリングのために、これまでに様々な電子ツールが開発されている。手指衛生遵守率改善を目的とした電子式手指衛生フィードバック装置の実用可能性と有効性を評価するため、評価者を盲検化した前向きパイロット研究を実施した。試験参加 1 か月目の手指衛生遵守率（37％）をベースライン値とした。2 か月目から 5 か月目の平均遵守率は、それぞれ 43％、44％、45％、および 49％であった（P ＜ 0.001）。この電子式装置は実用可能であり、手指衛生遵守率の若干の改善が得られた。このような電子式装置の効果をより大きな規模で検証するための研究が実施される予定である。

監訳者コメント：
手指衛生遵守率の向上とその維持は感染管理において極めて重要かつ果てしない課題である。この論文で紹介されている電子式ツールは、部屋毎に設置する機械が 1 台 500 ドル、スタッフがポケットに入れるデバイスが 1 台 150 ドルである。手指衛生遵守率モニタリングは人的な直接観察法から、本論文で使用されたセンサー式、消毒薬の使用回数や使用量から推測する方法と様々であるが、どれも導入当初は遵守率向上に貢献するものの、見られていることになれてしまうと、その効果も徐々に薄れていく。つまりは、ホーソン効果が最も有効な遵守率向上のツール（？）なのであろうか。いずれにしても、感染管理に費やす費用を何に投資するのか、そのアセスメントが感染管理担当者の悩みどころである。
