本研究では、完全埋め込み型中心静脈アクセスポートデバイス Port-A-Cath(Smith Medical 社、St. Paul、MN、米国)の管理が、カンジダ血症を併発した癌患者 98 例の転帰に及ぼす影響を調べた。Port-A-Cath 留置は不良な転帰と有意に関連しており、これは他の重要な予後不良因子(ブレイクスルーカンジダ血症、Acute Physiology and Chronic Health Evaluation[APACHE] II スコア ≧ 21、および Eastern Cooperative Oncology Group performance status[ECOG PS]高値[3 ~ 4])とは独立していた。しかし、カテーテル関連カンジダ血症の確定診断がなされていない患者、完全静脈栄養法を使用していない患者、ECOG PS または APACHE II スコアが不良ではない患者、および敗血症性ショックのない患者に対しては、Port-A-Cath 留置を考慮することが可能である。

監訳者コメント:
本論文では、カンジダ血症と診断されたときのポートの抜去にフォーカスしている。対象となった 98 人のほとんどに抗真菌薬が投与されていたが、投与のタイミングは中央値で 3 日(範囲は、カンジダ血症発症の 19 日前から 17 日後)であった。血液培養陽性から抗真菌薬投与開始までの日数が予後に大きく影響するという報告もあり、結果への影響が気にかかった。
なお、ブレイクスルー感染症とは、抗真菌薬の投与中に生ずる新たな真菌感染症を意味する。
