Demographic screening for MRSA may compromise the effectiveness of ring fencing on a joint replacement unit

関節置換術病棟の囲い込み（ring fencing）^※によって手術部位感染が減少することが報告されており、オーストラリアおよび英国の保健当局はこれを推奨している。囲い込みの対象病棟へのメチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）保菌患者の入院を避けるための方法として、人口統計学的リスク評価が適切であるかどうかは明らかではない。そこで、シドニー郊外の病院の関節置換術病棟への入院患者 250 例をスクリーニングしたところ、人口統計学的リスク評価の基準に合致した患者の 2.8％に MRSA 保菌が認められた。人口統計学的リスク評価は、身体的な MRSA スクリーニングの代替法として適切ではなく、囲い込み戦略の効果を損なう。
