Postprocedural discitis of the vertebral spine: challenges in diagnosis, treatment and prevention

背景
術後椎間板炎は比較的まれであるが、極めて重大な臨床転帰をもたらすことがある。
目的
術後椎間板炎の診断、管理、および予防の現況をレビューすること。
方法
過去 20 年間に英語で発表された文献を、種々の適切な検索語を用いて検索した。
結果
診断にあたっては、臨床的特徴、微生物学的検査結果、画像検査、および炎症性マーカーを使用するべきである。不必要な抗菌薬使用を回避するため、また標的を絞った治療を行うために、感染症の確診に注力するべきである。外科的デブリードマンや感染巣コントロールは治療に不可欠であり、これによって抗菌薬療法の指針となる診断用検体を得ることができる。培養陽性の場合は、抗菌薬感受性検査の結果に基づいて抗菌薬療法を行うべきである。抗菌薬療法に関する確定的なガイドラインは存在しない。経験的治療法として、キノロン系やクリンダマイシンなどとフシジン酸やリファンピシンとの併用が処方されている。静注剤から経口剤への早期変更と、6 週以上の抗菌薬療法が推奨される。予防法として、周術期の抗菌薬予防投与、良好な手術手技、および適用可能な場合の低侵襲手術が挙げられる。
結論
現時点で利用可能な情報の多くは不十分であり、良質な臨床試験が行われていない。日常的な培養の代替法、および予防策としての局所抗菌薬療法の意義に関するさらなる研究が必要である。

脊椎手術は開心術、人工関節置換術と並んで、待期手術での感染リスクは低いが感染すると重篤な有害事象につながるため、しっかりした感染予防策が重要である。
