2 病院における多剤耐性緑膿菌(Pseudomonas aeruginosa)アウトブレイク:汚染された病院排水システムとの関連

背景
VIM-メタロβ-ラクタマーゼを産生する多剤耐性緑膿菌(multidrug-resistant Pseudomonas aeruginosa;MDR-P)は、感染制御に関する新たな問題となっている。これらによる感染の多くは感染源が不明であるが、水道水を含む環境汚染に関連する緑膿菌の病院アウトブレイクが報告されている。
目的
コリスチンのみに感性を示す MDR-P による 2 件のアウトブレイクについて報告し、病院の排水システムに本菌が生息している可能性を示すこと。
方法
記述疫学、環境調査・微生物学的サンプル採取、および細菌株の分子タイピングを組み合わせて、アウトブレイクの調査を行った。
結果
英国の 2 病院でアウトブレイクが発生し、これらは異なる遺伝子型の MDR-P によるものであった。一方のアウトブレイクは病院全体に広がり、患者 85 例が関係していたが、他方は単一の診療部門の 4 例に限られていた。それぞれのアウトブレイク時に広範囲の環境サンプル採取を実施したところ、MDR-P が排水システムのみから認められた。環境調査および建物の管理記録の調査から、流し・シャワー・トイレの設計不良、汚物洗浄処理室(sluice)付近での清潔な物品の保管、および排水管の頻繁な詰まりや漏れなどの、臨床エリアの汚染に寄与した可能性のある多数の因子が明らかになった。詰まりの原因は、紙タオル、患者のワイプ、またはベッドパンのマセレータの不適切な使用であった。制御対策として、流しとトイレを洗浄が容易で水はねしにくいモデルへ交換すること、詰まりや不適切な保管を減らすための職員教育、清掃手順の見直し、あふれを減少させるためのシャワーの流量低下などを実施した。これらの対策により症例数が大幅に減少した。
結論
これらのアウトブレイクにより、病院の排水システムが MDR-P やその他の院内病原体のリザーバとなる可能性があることが示された。
