Hospital-acquired listeriosis associated with sandwiches in the UK: a cause for concern

背景
院内感染リステリア症のアウトブレイクの報告は多くはないが、依然として公衆衛生上の重大な問題である。
目的
病院で発生しているリステリア症のアウトブレイクに対する認識の向上を図ること、および脆弱な患者に対する食品由来リステリア症のリスクを最小限にするために取ることができる行動について述べること。
方法
英国健康保護局の全国的サーベイランスシステムに報告された食品由来のリステリア・モノサイトゲネス（Listeria monocytogenes）によるリステリア症のアウトブレイクおよび発生について調査を実施し、病院に関連する事例を抽出した。アウトブレイクおよび発生の背景、媒介食品、アウトブレイクの寄与因子、および問題の原因を明らかにするため、データ解析を行った。
結果
1999 年から 2011 年に英国で発生した食品由来リステリア症のアウトブレイクのほとんど（11 件中 8 件、73％）は、病院で購入した、または病院が提供したサンドイッチが関連していた。これらのアウトブレイクでは、包装済みサンドイッチおよびサンドイッチの製造環境から L. monocytogenes の感染性サブタイプが繰り返し検出された。5 件のアウトブレイクでは、病院による食品のコールドチェーンの管理違反も発生していた。
結論
これらのアウトブレイク事例から、L. monocytogenes に汚染されたサンドイッチが脆弱な患者らの重度の感染症の原因となり得ることが判明した。健康に危害を及ぼすような L. monocytogenes の摂取の可能性を最小限にするためには、サンドイッチ製造時や病院内での L. monocytogenes の制御が不可欠である。病院にサンドイッチを配送する製造業者は、製造時にサンドイッチに L. monocytogenes が存在しないようにすることを目指すべきであり、また病院が作成する食品安全管理システムにおいては食品のコールドチェーンに完璧を期するべきである。
