メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）のムピロシン高度耐性のために、鼻腔除菌プロトコールにおけるムピロシンの意義が損なわれている。2010 年に、著者らの 3 次病院のムピロシン高度耐性率を明らかにするための研究を実施した。当院の MRSA のムピロシン高度耐性率は 11％であった。スクリーニングにより検出された 24 の分離株では、ムピロシン高度耐性の遺伝子型を用いた検出法と表現型を用いた検出法の結果に完全な一致がみられた。

ムピロシン信仰はすでに限界にきている。耐性菌も各国で発見されており、予防にシフトしていくべきである。
