Understanding the epidemiology of MRSA in Europe: do we need to think outside the box?

背景
菌血症を指標としたメチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）感染の疫学は、欧州では顕著な地理的パターンが認められている。MRSA 菌血症有病率は北欧諸国では低く、中欧にかけて増加し、地中海地域で最大となる。これは感染制御・抗菌薬管理プログラムの実施レベルの相違に起因するとされてきたが、このような違いの理由は明確にはされていない。
目的
欧州における MRSA の疫学に対する各国の文化的特性の影響について調査すること。
方法
2010 年に EARS-Net サーベイランスネットワーク参加国の MRSA 菌血症有病率中央値を算出し、Hofstede ら^※による各国の文化的特性スコアとの関連を評価した。
結果
MRSA 菌血症有病率と、文化的要素である不確実性の回避（uncertainty avoidance；UAI）、男性らしさ（masculinity；MAS）、および権力格差（power distance）との間に有意な関連が認められた。重回帰モデルからは、UAI、MAS、および短期志向（short-term orientation）を組み入れたモデルにおいて有意な関連が認められた（補正 R² ＝ 0.475、P ＜ 0.001）。このモデルにより、2006 年から 2010 年に欧州のいくつかの国でみられた MRSA の動向が予測できることが示された。
結論
感染制御・抗菌薬管理プログラムの実施にあたっては、行動変化が必要となることが多い。文化的特性は医療従事者の認識と価値観を左右する主要な因子であると考えられ、したがって、プログラムの遵守と理解のためには極めて重要である。地域の文化的背景を感染制御・抗菌薬管理に関する取り組みに反映させることによって、成功に至る可能性が高くなると考えられる。
