Hot and steamy: outbreak of Bacillus cereus in Singapore associated with construction work and laundry practices

背景
シンガポールの National University Hospital でバチルス・セレウス菌群（Bacillus cereus group）による侵襲性感染症が突如増加したことを受けて、調査を実施した。
目的
アウトブレイクの調査と管理について述べるとともに、その後の管理の維持に関する問題点を報告すること。
方法
臨床サンプルから B. cereus group が回収された全患者を対象として、症例調査を実施した。広範囲の環境サンプルを採取し、病院の換気設備、院内清掃、および洗濯プロセスの精査を行った。
結果
病院に隣接して大規模な建設工事が行われていた 6 か月間に、171 例の患者から B. cereus group が回収された。大部分の患者が菌血症であり（171 例中 146 例、85.4％）、171 例中 46 例（26.9％）はバンコマイシンやその他の介入による長期治療が必要であった。サンプル採取の結果、隔離室や空調設備のある病棟を含めて、病院内の空気中に広範囲に拡散していることが確認された。病院のリネン類は高度に汚染されており（7,403 cfu/cm²、95％信頼区間［CI］6,349 ～ 8,457、サンプル採取したタオル 30 枚のデータ）、不適切な密封性ビニール袋で保管した場合（4,437 cfu/cm²、95％CI 3,125 ～ 5,750）は、通気性のある布袋で保管した場合（166 cfu/cm²、95％CI 76 ～ 256）と比較して汚染が高度であった（P ＜ 0.001）。病院のリネン類やタオルの洗濯プロセス、移動、および保管の改善、漂白剤を使用した環境清掃、および病院全体の換気設備の改善などの介入を実施した。症例数は 3 か月以内でベースラインレベルに回復したが、洗濯プロセスを緩和したところ、再度増加した。
結論
シンガポールの当病院に隣接して行われた建設工事に伴い、本稿に述べたアウトブレイクの原因菌と推定される Bacillus 属菌による空気および環境の高度の汚染が引き起こされた。洗濯プロセス改善の継続が不十分となったことにより、症例数が、特に免疫不全患者において再度増加した。
