蒸気化過酸化水素によるアシネトバクター・バウマニー(Acinetobacter baumannii)感染アウトブレイクの制御
Control of an outbreak of Acinetobacter baumannii infections using vaporized hydrogen peroxide

背景
多剤耐性アシネトバクター・バウマニー(multidrug-resistant Acinetobacter baumannii;MRAB)は重大な院内感染病原体であり、無生物表面で長期間生存するという特性を有するためアウトブレイク制御が困難である。
目的
MRAB による 2 件の院内アウトブレイクを分析し、その疫学とカルバペネム耐性機序を明らかにするとともに、蒸気化過酸化水素(VHP)による表面消毒の有効性を評価すること。
方法
集中治療室(ICU)2 室の患者から MRAB 株を分離した。E-test を用いて抗菌薬感受性試験を行った。優勢に認められる A. baumannii 由来カルバペネマーゼをPCR 法を用いて検出した。疫学的分類を rep-PCR法(DiversiLab)とパルスフィールド・ゲル電気泳動により行った。感染がみられた ICU を VHP により汚染除去した。
結果
2009 年 1 月から 2010 年 9 月に 28 例の患者から MRAB が分離された。全分離株がシプロフロキサシン耐性およびゲンタマイシン耐性を示した。21 株はカルバペネム系にも耐性であった。カルバペネム耐性は主に獲得型 OXA-23-like 酵素と関連していた。遺伝子型判定により 3 つのクローンが判明し、優勢なクローンはinternational clone(IC)2 型に相当するものであった。分離株のタイピングから、感染源が単一ではない多発性のアウトブレイクであることが示唆され、優勢なクローンが ICU の患者間で水平伝播したと考えられた。厳重な隔離、職員の教育、手指衛生、および VHP を用いた表面の汚染除去を含む厳格な感染制御対策を併用することにより、アウトブレイクは終息した。
結論
本研究の結果から、MRAB アウトブレイクの制御には、厳格な感染予防・制御対策を VHP による汚染除去と併用することが重要であることが確認された。
