Rapid detection of intestinal carriage of Klebsiella pneumoniae producing KPC carbapenemase during an outbreak

KPC 型カルバペネマーゼ産生肺炎桿菌（Klebsiella pneumoniae）腸内保菌の 2種類の迅速検出法、すなわち直腸スワブからの DNA抽出物の PCR 法による増幅（K-PCR 法）と、メロペネムディスクおよびメロペネム＋3-アミノフェニルボロン酸ディスクを設置したマッコンキー寒天培地と直腸スワブによる直接平板法（直接 KPC スクリーニング法、DKST法）について述べる。アウトブレイク中に患者 65 例から得た合計 101 サンプルを対象として、K-PCR 法および DKST 法の評価を行った。DKST 法の感受性は K-PCR 法よりも低かったが、感染・保菌患者に多くみられる高レベルの保菌は検出可能であり、しかも極めて安価かつ実施が容易で、基本的な設備しか必要としない方法であった。
