背景
嚢胞性線維症患者に使用した非侵襲的人工呼吸器の細菌汚染の想定リスクについて、現時点でエビデンスはほとんど得られていない。
目的
本研究の目的は、当地域の成人嚢胞性線維症専門医療施設における非侵襲的人工呼吸器の細菌汚染レベルを明らかにすることである。
方法
緑膿菌(Pseudomonas aeruginosa)またはバークホルデリア・セパシア(Burkholderia cepacia)complex(セパシア菌群)による慢性感染症を有する嚢胞性線維症患者が最近使用した非侵襲的人工呼吸器 7 台を対象として、外側と内側の合計 7 か所からスワブ採取を行った。2 台に対しては、患者が使用してからスワブ採取までの間にエチレンオキサイド(EtO)滅菌を行い、5 台は EtO 滅菌は行わなかった。
結果
5 台の装置からのスワブ標本では環境微生物のわずかな増殖しかみられず、2 台のスワブ標本では大幅な増殖が認められた。いずれの装置からも、嚢胞性線維症に関連する感染症の起因菌は検出されなかった。
結論
今回の小規模な研究からは、嚢胞性線維症患者が使用した非侵襲的人工呼吸器に病原微生物汚染は認められなかった。著者らは、さらなる研究により非侵襲的人工呼吸器の細菌汚染についての評価を行うこと、また、今後の嚢胞性線維症に関連する感染制御ガイドラインにおいて、この問題を取り扱うことを提案する。

監訳者コメント:
人工呼吸器の使用後にどのレベルまで消毒・滅菌するのかということと、VAP の発生との関連性は関心の高いところである。この研究ではエチレンオキサイド滅菌後に採取したサンプルから菌が検出されていたが、滅菌法に問題がないと仮定すれば、サンプル採取時に環境菌を拾ったのであろう。医療施設での研究の難しさが伺えるとともに、研究デザインの重要性を考えさせられる論文である。
