Bloodstream infections caused by Stenotrophomonas maltophilia: a seven-year review

背景
ステノトロホモナス・マルトフィリア（Stenotrophomonas maltophilia）は重要な院内感染病原体であり、感染症の増加、特に免疫不全患者での増加の原因となっている。

目的
S. maltophilia 菌血症患者の臨床的および微生物学的特性について述べること。

方法
米国・ニューヨークの 2 つの 3 次ケア施設における 2001 年から 2007 年の 7 年間の S. maltophilia 菌血症患者 102 例を対象としてカルテレビューを行った。

結果
患者の内訳は、院内感染エピソード 79 例（77.5％）、医療関連感染ピソード 21 例（20.6％）、および市中感染エピソード 2 例（2％）であった。菌血症の感染源として最も頻度が高いものは、汚染された中心静脈カテーテルの 44 例（43.1％）であった。また、17 例（16.6％）は好中球減少性敗血症と関連があり、9 例（8.8％）は腹部、6 例（5.9％）は呼吸器が感染源であり、26 例（25.5％）では感染源は不明であった。ほとんどの患者（94.1％）が中心静脈アクセス器材を留置していた。単変量解析では、集中治療室入室、気管内挿管、敗血症性ショック、菌血症時の好中球減少症、またはエピソード発生前 30 日以内のカルバペネム系抗菌薬使用と、死亡との関連が認められた。多変量解析では、死亡と有意に相関する因子は、低血圧性ショックおよびエピソード発生前 30 日以内のカルバペネム系抗菌薬使用であった。分離株 102 株の多くは、トリメトプリム・スルファメトキサゾール（97.1％）、レボフロキサシン（92.9％）、セフタジジム（53.0％）、およびチカルシリン・クラブラン酸（49.2％）に対して in vitro で感受性を示した。

結論
本研究のデータは、S. maltophilia 菌血症患者の特性を示すとともに、これらの患者の血管アクセス器材を注意深く評価することの重要性を強調するものである。
