Microbial air monitoring in operating theatres: experience at the University Hospital of Parma

背景
手術室における空気中の微生物モニタリングは、関心が寄せられ議論の対象となっているテーマである。標準的なサンプル採取方法や許容可能とされる微生物量の閾値はない。
目的
従来の方法による換気を行っているイタリアの Parma 大学病院の手術室を対象として、無人の場合と業務中の場合の空気中の微生物汚染を 3 年間にわたり評価すること。
方法
29 の手術室から空気サンプルを採取した。能動的採取法および受動的採取法の両手法により細菌・真菌汚染を評価した。
結果
無人の手術室で記録された細菌量の中央値は、12 コロニー形成単位（cfu）/m³（四分位範囲［IQR］4 ～ 32）、および 1 IMA（空気の微生物汚染指標）（IQR 0 ～ 3）であった。業務中の手術室では、これらの数値は 80 cfu/m³（IQR 42 ～ 176）および 7 IMA（IQR 4 ～ 13）へ有意に上昇した（P ＜ 0.001）。記録された最大値は、無人の手術室で 166 cfu/m³ および 8 IMA、業務中の手術室で 798 cfu/m³ および 42 IMA であった。能動的サンプル採取法と受動的サンプル採取法を組み合わせたところ、無人の手術室から採取したサンプルでは 39.13％、業務中の手術室では 56.95％から真菌が分離された。3 年間にわたる研究期間中に、微生物汚染は無人および業務中の手術室の両方で減少し、真菌が認められないサンプルの割合が上昇した。業務中の手術室では、受動的サンプル採取法と能動的サンプル採取法による微生物汚染量の間に有意な相関が認められた（P ＜ 0.001）。
結論
微生物モニタリングは、手術室の汚染の評価、および空気の質向上に有用な手法である。
