銀コーティングおよび非コーティングの血管内ニードルレス・コネクターの臨床使用での微生物学的比較

背景
血管内ニードルレス・コネクターの微生物汚染の可能性とその後の感染リスクについては現在、議論の対象となっている。
目的
臨床環境で銀コーティングコネクターおよび非コーティングコネクターを使用した場合の菌数を比較すること。
方法
本研究の対象は、臨床的管理の一環として中心静脈カテーテル(CVC)を必要とする血液悪性腫瘍患者 25 例であった。各患者の CVC を銀コーティングコネクターまたは非コーティングコネクターのいずれかに無作為に接続した。コネクターの使用前後に、2%(w/v)グルコン酸クロルヘキシジン含有 70%(v/v)イソプロピルアルコール含浸ワイプによりコンプレッションシールの汚染除去を行った。留置 4 日後に、銀コーティングコネクター 119 個と非コーティングコネクター 117 個から回収した菌数を測定した。
結果
外側のシリコン製コンプレッションシールから菌が検出された銀コーティングコネクターは 36 個(30.3%)、非コーティングコネクターは 41 個(35%)であった(オッズ比[OR]0.8、95%信頼区間[CI]0.47 ~ 1.39、P = 0.49)。一方、内側の流路から菌が検出された銀コーティングコネクターは 31 個(26.1%)、非コーティングコネクターは 55 個(47.0%)であった(OR 0.40、95%CI 0.23 ~ 0.69、P = 0.001)。また、検出された総菌数は銀コーティングコネクターのほうが非コーティングコネクターよりも少なかった(P = 0.001)。
結論
至適な汚染除去法を適用した銀コーティングコネクターを使用することによって、内腔を介するカテーテル関連血流感染リスクが減少すると考えられる。
