Outbreak of a South West Pacific clone Pantone-Valentine leucocidin-positive meticillin-resistant Staphylococcus aureus infection in a UK neonatal intensive care unit

背景
Pantone-Valentine 型ロイコシジン産生メチシリン耐性黄色ブドウ球菌（Pantone-Valentine leucocidin-positive meticillin-resistant Staphylococcus aureus；PVL-MRSA）は、市中獲得感染の原因菌として世界各国で高頻度で認められるようになっている。
目的
英国の地域新生児室で乳児 3 例と職員 3 例が関与した PVL-MRSA アウトブレイクについて報告する。
方法
PVL-MRSA の推定にはキノロン感性が有用であるが、当室の 2 株の PVL-MRSA を識別するためには毒素遺伝子のプロファイリングとシークエンシングが必要であった。
結果
有症状の乳児全 3 例および保菌職員 2 例（このうち 1 例は有症状）が、PVL-MRSA の南西太平洋（SWP）クローン（ST30）を保有していることが判明した。保菌職員の 1 例はフィリピンを訪問しており、アウトブレイクの発生源であると考えられた。標準的な感染制御策による制御を実施したが、MRSA 保菌が再発した乳児 1 例は 100 日を超える隔離が必要であった。
結論
本稿は、英国における SWP クローンによる新生児アウトブレイクの初の報告である。著者らの研究は、フィリピンと疫学的な関係がある医療施設の職員や患者が、今後も本菌を持ち込むリスクがあり得ることを示すものである。
