擦式手指消毒薬の使用量は手指衛生遵守率および基質特異性拡張型β-ラクタマーゼ産生腸内細菌科細菌（ESBL-PE）感染率と相関するか？

擦式アルコール製剤の使用量は、メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）または基質特異性拡張型β-ラクタマーゼ（ESBL）産生菌による院内感染の発生率と相関するかどうかを、後向き研究により評価した。擦式アルコール製剤の 1,000 患者入院日あたりの使用量は、2005 年から 2008 年にかけて毎年 8 L という大幅な増加がみられた。同期間の手指衛生遵守率は 55.6％から 70.9％に大幅に上昇した（P ＜ 0.0001）。このような改善がみられたにもかかわらず、ESBL 産生菌の発生率は過去 3 年間、一貫して上昇し、擦式アルコール製剤の使用量は、ESBL 産生菌による院内感染または手指衛生遵守率のいずれとも相関しなかった。

監訳者コメント：
これまで、擦式アルコール製剤（ABHR）使用量は手指衛生遵守の指標として有効と考えられてきたが、本研究では、ABHR 使用量の増加にもかかわらず ESBL 産生菌の検出が増加した理由として、そもそもの使用量算出の方法や、増加の解決は手指衛生だけでなく、抗菌薬の使用や保菌圧の関連の可能性を挙げている。
