蒸気化過酸化水素およびエアロゾル化過酸化水素を用いる室内消毒装置の滅菌効果、散布能力、および安全性

背景
過酸化水素を用いる 2 種類の室内消毒装置の直接比較を行った。
目的
蒸気化過酸化水素(HPV)室内消毒装置(Clarus R、Bioquell、Andover、英国)とエアロゾル化過酸化水素(aHP)室内消毒装置(SR2、Sterinis[現在はGlosairとして販売]、Advanced Sterilization Products[ASP]、Johnson & Johnson Medical Ltd、Wokingham、英国)の滅菌効果、散布能力、および安全性を比較すること。
方法
4 log および 6 log のゲオバチルス・ステアロサーモフィルス(Geobacillus stearothermophilus)をバイオロジカルインジケータとして、メチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus;MRSA)、クロストリジウム・ディフィシル(Clostridium difficile)、およびアシネトバクター・バウマニー(Acinetobacter baumannii)約 106 個を接種した検査ディスクを院内で作製し、滅菌効果の検査を行った。過酸化水素の漏出を携帯型検知器で検出することにより安全性を評価した。サイクル開始から 2 時間後の室内 3 か所の過酸化水素濃度を携帯型センサーで測定し、散布能力を評価した。
結果
バイオロジカルインジケータおよび院内で作製した検査ディスクに対する滅菌効果は、HPV ではおよそ 6 log の減少であったのに対して、aHP ではおよそ 4 log 未満であった。測定箇所による滅菌効果のばらつきが aHP 装置では認められたが、HPV 装置では認められなかった。製造会社の操作マニュアルに従って部屋のドアを密閉しない状態で aHP を運転した場合の過酸化水素の漏出は、2 時間以上にわたって短時間曝露限界(2 ppm)を超えた。HPV 装置の操作マニュアルに従って部屋のドアをテープで密閉した場合の過酸化水素の漏出は、両装置とも 1 ppm 未満であった。サイクル開始から 2 時間後の室内の過酸化水素濃度の 4 サイクル平均値は、HPV 1.3 ppm(標準偏差[SD]0.4 ppm)、aHP 2.8 ppm(SD 0.8 ppm)であった。aHP のサイクルでは測定値が 2 ppm 未満となることはなかった。
結論
HPV 装置のほうが安全かつ速効性に優れ、微生物の不活化効果が高い。
